There are various forms for various organizations.
1) There is one for the DMV (Reg 1000) for your clean air sticker. There are no limits to the number of Clean Air Stickers that will be issued to NGV's through the new Sunset Date (Dec 31, 2014)
2) Letter of intent to Socal Gas if you are intrested in a Phill.
3) Once you have a VRA , like Phill or a FMQ you fill aot the rate change request for SoCal gas.**
4) IRS Form 8910 is to take the $4000 tax credit. (Google the Form # to get the current year form.)
5) IRS form 8911 is the $2000 property refueling tax credit for the house.* (Same as #4 above)
The IRS forms 8910/8911 should be downloaded directly from the IRS website. They change annually and undergo revisions. This way you know you have the latest version.
* The property refueling credit was raised from $1000 to $2000 in January 2009 as part of the recovery legislation.
** If you live in a "Hot Gas" area and are turned down for a home fueling unit, such as the Phill, contact me. There is now a way to obtain a waiver from CARB. No area is now unable to have home refueling.
I have put them all here for you with the exception of the IRS forms, as they change yearly and your tax person can simply go to www.irs.gov and do a form search for them.
